Frequently Asked Questions

1.        Is Bridges to Care health insurance?
Answer: No.  Bridges to Care is a program that links persons to clinics/providers that will serve people without health insurance and adjust charges based on their ability to pay.

2.       Can I register with Bridges to Care if I live outside of Davidson County?
Answer: No. Bridges to Care is only for residents of Nashville/Davidson County.  However, many of the clinics in the Bridges to Care network will serve persons from other counties.

3.       Can I choose where I receive primary health care services?
Answer: Yes. You may choose any of the clinics in the Bridges to Care network.  When filling out the application, you must choose the clinic you wish to use. This will become your “medical home” where you will go for primary health care needs.

4.       Can I change my medical home?
Answer: Yes.  Although we want you to make an informed decision and stick with your medical home, you can change your choice of medical home by calling 340-0573.

5.       What are your hours of operation?
Answer: You can register online at any time.  To register in person, the Health Department locations are open 8 am to 4:30 pm, Monday thru Friday.

7.       If I am employed can I be a part of Bridges to Care?
        Answer: Yes, the only eligibility criteria are that you live in Davidson County and are
        uninsured.

8.       What do I have to bring to enroll in the program?
Answer:  You need to bring proof of your residence.  A driver’s license or some document that shows your address is sufficient.

9.       What if I don’t have a social security number?
Answer:  You can still register for Bridges to Care at any Health Department location or on this web site.

10.    Do I need an appointment to register in Bridges to Care?
Answer: No.  Registration is an easy process that usually takes 5 to 10 minutes.  You can enroll at any time online or at any Health Department location, Monday through Friday from 8 am-4:30 pm.
