Bridges to Care
Plus
Bridges to Care Plus, administered by the Nashville
Academy of Medicine, links Bridges to Care
participants to physician specialists upon referral by
their primary care physician. Patients pay $10 per
visit. Help with prescriptions is provided. If the
specialist determines that inpatient or outpatient
services are required, these services are made available
at no charge to the patient.
Requirements to Participate:
Must be registered as a Bridges to Care
participant
Must be referred to the program by a Bridges to Care
primary care health clinic
Must have family income below 200% of the federal
poverty level.
