Bridges to Care
Bridges to Care links
uninsured residents of Davidson County to a network of
low-cost community clinics that charge for service’s
based on the patient’s ability to pay. The Bridges
to Care network includes an array of primary care
providers, dental care providers, mental health
providers, substance abuse providers, referrals to
specialty care, access to prescription medications, and
transportation to medical appointments.
History
Since its inception in 2002, Bridges to
Care has linked more than 46,000 uninsured Davidson
County residents to a medical home where they receive
primary health care at a cost based on their income.
BTC has filled over 182,000 prescriptions and provided
more than 6,800 transports to clinic appointments.
Partnerships
Bridges to Care was created by the “safety net”
providers in Nashville including the hospitals with
Emergency Departments. All are now members of the
not-for-profit organization known as the Safety Net
Consortium of Middle Tennessee. The Consortium
contracts with the Nashville Academy of Medicine for
specialty care and Kroger for generic prescriptions.
Metro Public Health Department fills prescriptions for
some Bridges to Care registered patients and manages the
Bridges to Care website. Saint Thomas Health Services
Fund provides fiscal and legal support for the
Consortium |
