About Bridges to Care

Bridges to Care (BTC) links uninsured residents of Nashville to a network of some 35 safety net primary health, dental, mental health, and substance abuse clinics that serve patients based on their ability to pay.  BTC also provides help with prescription medications and transportation.

Bridges to Care has Care Coordinators stationed at the following locations to screen and enroll persons into the program.

Requirements to Enroll

·        Have no health insurance
·        Live in Nashville/Davidson County
·        Complete the Bridges to Care application
·        Sign a release of information
·        Provide proof of income
·        Provide proof of Davidson County residency
·        Complete interview with BTC Care Coordinator
·        Choose a medical home with the help of the Care Coordinator
·        Pay fees based on income and family size at participating clinics
·        Go to hospital emergency departments only in an emergency

About Bridges to Care Plus

Bridges to Care Plus, administered by the Nashville Academy of Medicine, links Bridges to Care participants to physician specialists upon referral by their primary care physician.  Patients pay $10 per visit.  Help with prescriptions is provided.  If the specialist determines that inpatient or outpatient services are required, these services are made available at no charge to the patient.
Requirements to Participate
·        Must be a Bridges to Care enrollee
·        Must be referred to the program by a Bridges to Care primary care health clinic
·        Must have family income below 200% of the federal poverty level
